Record Provenance

The Patient Chart displays metadata about the origin and source of each clinical record so you can verify where the information came from and assess its reliability. Provenance is derived from FHIR resource metadata, contributing documents, and national network transactions.

Source Information

The viewer shows provenance details at the section, row, or card level depending on the type of data. When available, each entry includes:

  • Source organization name (typically the document sender)
  • Date the record was authored or retrieved
  • Reference to the original document or encounter
  • External identifier or accession number (for labs and imaging)

Visual markers, such as source organization names or document icons, distinguish between document-based and inferred data. Provenance tags appear inline next to the data or in a collapsible detail view. Hovering over the source label or expanding a row reveals additional details, such as the originating network, document type, or authoring provider.

Document-Based Attribution

For data parsed from clinical documents (such as CCDA, C-CDA on FHIR, or PDF attachments), the viewer links each data element to its source document. Selecting a record opens the full document in the viewer’s right pane for side-by-side review. This allows you to assess individual entries within the context of the originating document. DocumentReference resources are used to maintain these links.

FHIR Resource Metadata

FHIR attributes such as meta.source, meta.lastUpdated, and Provenance.target are used to determine display labels and attribution. In some cases, Health Gorilla enhances provenance with additional network context, such as which endpoint provided the document or what exchange transaction triggered the retrieval.

Network and Facility Details

If a record was retrieved from a national network (such as Carequality or eHealth Exchange), the viewer may include the name of the participating facility or health information exchange (HIE), along with network-specific identifiers. These are derived from organization metadata and audit trails maintained during document exchange. This contextual information is available in the provenance hover panel or detail view when applicable.
