Overview
The SMART on FHIR Patient Chart allows you to securely display a patient’s longitudinal medical history using real-time data from Health Gorilla’s Patient360 solution. It integrates directly with EHRs, care coordination platforms, and support portals through a standards-based SMART on FHIR launch. The chart presents normalized FHIR data alongside external documents in a structured, read-only interface without storing any data locally.
You can embed the chart or launch it via redirect, using standard FHIR context parameters such as patient or encounter. Role-based access controls are enforced through OAuth scopes, user roles, and tenant-level configuration.
Capabilities
The chart offers the following key features to support clinical and operational workflows:
- Launches using SMART on FHIR with OAuth 2.0 and FHIR context
- Retrieves data in real time from national networks using Patient360
- Enforces access based on role, scopes, and consent
- Displays structured clinical data and external documents
- Supports both embedded and redirect-based workflows
- Requires no local storage or custom rendering
Use Cases
You can use the chart to enable the following workflows across your organization:
- Open a chart from within an EHR during a clinical encounter
- Embed the chart in a care management or intake platform
- Allow support staff to review records without full EHR access
- Access longitudinal history for referrals or case review
- Enable Patient360 viewing without building a custom viewer
